New Patient Experience
At Aligned Chiropractic, your first visit will begin with a warm smile and personal greeting! Your new patient experience spans two visits, allowing our doctor the time needed to carefully review your exam findings
Your first visit begins with a detailed health history and physical examination, followed by spinal X-rays. Most first visits are completed in one hour, but please allow 1.5 hours in your schedule.
As a friendly reminder, please avoid wearing clothing with metal for your X-rays. We’ll have exam clothing available if needed. We recommend loose, comfortable clothing and shoes that are easy to slip on and off.
If you have MRI images for the doctor to review, we kindly ask that you bring them to your first visit on a CD rather than providing access through a patient portal on a cell phone.
On your second visit, Dr. Green will review your exam results, answer any questions, discuss your personalized care plan, and perform your first spinal adjustment. Most second visits are completed in 45 min, but please allow one hour in your schedule.
If you will not be able to make your appointment, we request 24 hours notice so that another person in need of the doctor's time may be helped.
New Patient Insurance

If you have insurance you hope to utilize in our office:
Option 1: Once you select a new patient appointment time, you will receive a text request for a picture of your insurance card. If you would like to discuss your finances before your visit, please text the picture of your card and reply ESTIMATE. A team member will reach out to you once your benefits are verified.
Option 2: Call our office to provide us with your insurance company name, ID number, date of birth, the subscriber name (and subscriber's date of birth if the subscriber is not you), and the phone number on the back of your card for provider services, or member services.
New Patient Medicare
If you are 65 or older or have Medicare, please email our New Patient Coordinator at frontdesk@MySpinesAligned.com BEFORE filling out the online New Patient Forms.
We do not file with Medicare, or any secondary insurances affiliated with Medicare coverage. Due to Medicare laws regarding chiropractic care, we can NOT provide "medically necessary" care for you in our office. If you have opted out of Medicare, we MAY be able to provide medically necessary care for you. Please call our office.
